Sassuolo hosted AC Milan in their Serie A match at the Stadio Alberto Braglia.
The visitors started well as goals from Robinho and Mario Balotelli gave them a 2-0 lead inside 13 minutes.
Domenico Berardi quickly pulled one back for Sassuolo and added two more before half time to turn the game around.
The striker scored his fourth at the start of the second half, and although Riccardo Montolivo pulled a late goal back for Milan, the home side held on to record a famous win.
