Chelsea attacker Eden Hazard could reportedly leave Stamford Bridge in the January transfer window.

Hazard has been in fine form for Chelsea this term, managing eight goals and four assists in 15 appearances in all competitions, with seven of those strikes coming in the Premier League.

The Belgium international - a long-standing Real Madrid target - will see his Blues contract expire at the end of next season, however, and admitted during the summer that he 'dreams of moving to the Bernabeu'.

According to OK Diario, Chelsea have accepted that they are likely to lose Hazard, and the Belgium international could even head to Madrid at the start of 2019.

Los Blancos failed to replace Cristiano Ronaldo during the summer transfer window, and their shaky start to the season continued with a 3-0 defeat at Eibar on Saturday afternoon.
