Barcelona midfielder Xavi has come on in the second half of the 2015 Champions League final against Juventus for what will be his final appearance for the Catalan outfit.

The 35-year-old, who has played more than 750 times for Barcelona's first team, will move on to Al-Sadd this summer.

The midfielder started the European Cup final on the bench, but replaced Andres Iniesta with little over 10 minutes remaining.

As it stands, Barcelona, who are chasing a treble, lead 2-1 in Berlin.

Xavi has won eight league titles during his time with the Catalan outfit, and is chasing a fourth Champions League crown in Berlin.
